Read moreFuneral policy beneficiary nomination form
As you may be aware, you have a funeral policy, administered by Momentum, linked to your medical aid membership. This policy will pay out a benefit upon your or your dependants’ passing to assist with covering some of the funeral costs. Due to recent legislative changes, it has now become compulsory for all members to submit a beneficiary nomination form. This is a new requirement and you would not previously have completed a nomination form for this policy. This nomination form details who the proceeds of the policy should be paid out to in the event of your passing. Failure to have completed a duly signed beneficiary nomination form will result in the proceeds of this policy being paid into your estate and not to your next of kin, resulting in untimely delays. Please click "View form" below to complete and sign the nomination form and return it via email to [email protected]. The nomination form will be kept on file until such time as it may be needed. Please also make sure to keep a copy on file for your own records.
