The tables below list the chronic conditions covered on the Primary Option. Please note:
- Registration on the Chronic Medication Programme is required for benefits in respect of these conditions.
- Medication for the treatment of these conditions is subject to the network chronic medicine formulary.
- Not all medication for the conditions are covered on the network chronic medicine formulary.
- Should your medication not be on the formulary, you may need to visit your network general practitioner to discuss alternatives.

How to use the Formulary Look-up page to search for medication
On the top, left-hand menu, the Scheme name will be preset to CCH PICK N PAY. Below this, you will see the 'Option' drop-down menu, where you can choose to search for acute or chronic medication.
Acute Medication formulary
To find medication that is covered on the CareCross acute medication formulary, select the option called Carecross Health Acute.
